To register for an upcoming EIPA, please download the registration form for the date of the exam you wish to take. Complete the registration form and demographic information (leaving questions 3-6 blank) then mail it, with your payment of $250 (Please note the registration fee will increase from $250 to $275 on January 1, 2010) made payable to TAESE/USU.

Registration closes 30 days prior to the testing date. You will receive a letter and email from TASK12 confirming your registration, informing you of your test time, and giving you other information about the exam. If you have any questions please feel free to contact Cheryl Sheffield (cheryl.sheffield@usu.edu or 435-752-0238 x 13).

Note: If you do not submit the demographic information, payment or contact information for who will be paying, or if you do not circle your evaluation material, your registration will not be accepted.

 
Times fill up quickly so you will want to register early.
